Which one should a systematic review use?

PubMed (via MEDLINE) as a required, citable primary database, per PRISMA/Cochrane methodology -- its documented, reproducible syntax is what a reportable search strategy depends on. Google Scholar is commonly added as a supplementary grey-literature check, not a primary source, precisely because its syntax and inclusion criteria aren't documented the same way.
Does Google Scholar cover PubMed's content?

Largely yes, plus much more -- Google Scholar's crawl picks up most PubMed-indexed journal content along with preprints, theses, and non-MEDLINE-indexed material PubMed excludes. It does not, however, offer PubMed's MeSH indexing or documented syntax on top of that broader coverage.
Why does PubMed's own "Cited by" count look lower than Google Scholar's?

PubMed's native "Cited by" only counts citations from articles deposited in PubMed Central (PMC). Google Scholar counts citations from any document in its much broader index, including preprints and non-peer-reviewed material. NIH's iCite tool gives a fuller PubMed-based count, but it's still limited to citations from other PubMed articles.
Is there an official Google Scholar API to automate a comparison against PubMed's E-utilities?

No. PubMed has a free, official API (E-utilities). Google Scholar has never offered one; any product calling itself a "Google Scholar API" is an unofficial, unauthorized scraping service.
