Editorial commentary
Milestones are distinct from deliverables: a milestone is a point in time and a state of progress, while a deliverable is a tangible output. Horizon Europe formalises milestones in Annex 1 (the description of action) with named milestones, due dates, means of verification, and responsible WPs. Milestones often trigger management actions (e.g., go/no-go decisions, milestone-payment release, sponsor reviews). Missed milestones must be reported and may require remedial plans.
What it means on the award ledger and agreement
In a milestone-based agreement, each milestone in the grant agreement or contract is tied to a specific means of verification and, often, a reporting or payment checkpoint. Reaching a milestone does not by itself move money on a standard cost-reimbursement award — funding there still tracks actual expenditure — but on a fixed-price or hybrid instrument, milestone achievement is exactly what authorizes a milestone payment. Missing a milestone does not automatically require repayment, but it must be disclosed in the next progress report and can require a corrective or remedial plan, an amendment to the work plan, or, in a phased award, a formal go/no-go review before further funding is released.
How it interlocks with related terms
A milestone is a checkpoint; a project deliverable is the tangible thing produced (a report, dataset, or prototype) that may or may not coincide with a milestone date; and a milestone payment is the funding action a milestone can trigger under a fixed-price mechanism. On a longer multi-year award, milestone progress is also the evidence base a mid-term review draws on to judge whether the remaining plan is realistic.
Who decides
The work package leader or PI proposes evidence that a milestone has been achieved (the means of verification specified in the agreement); the sponsor’s project or programme officer — or, at NIH, the Grants Management Officer in consultation with the Program Officer — reviews and formally accepts it. On some fixed-price or industry-sponsored awards, an independent reviewer or the sponsor’s technical monitor verifies achievement before payment is authorized.

What happens if a milestone is missed? It must be reported in the next progress or financial report; the sponsor may require a written remedial plan, an amended timeline, or — on a fixed-price award — may withhold the associated milestone payment until it is achieved.
