Why a blood bank/transfusion service checklist is different from a general lab checklist
A hospital blood bank or transfusion service is not just another section of the clinical laboratory with a different test menu. It runs three distinct operational lanes at once — a donor-draw or specimen-collection area, a component-processing and storage bench, and a bedside-facing transfusion-support function that has to respond within minutes when something goes wrong at the patient’s IV line. A general lab supply list built around instrument reagents and pipette tips misses most of what this department actually consumes day to day: collection-set consumables for draws, filtration-adjacent supplies for leukoreduced components, and a stocked, staff-known kit for the first few minutes of a suspected transfusion reaction. This checklist is organized around those three lanes.

Donor-draw-site and collection-set supplies
A blood bank’s own draw activity — autologous/directed donations at facilities that still collect them, or more commonly, specimen collection for type and screen, crossmatch, and antibody workup — has stocking requirements shaped by two things a general phlebotomy tray doesn’t have to account for: the higher failure cost of a repeat stick on a patient who needs blood urgently, and chain-of-custody labeling that has to be correct at the moment of draw, not corrected afterward.
- Winged (butterfly) infusion sets in a range of gauges — 21-23 gauge for standard adult draws, smaller gauges held in stock for pediatric, geriatric, oncology, and other difficult-access patients. - Pre-transfusion specimen tubes correctly matched to the testing being ordered — a pink-top (K2EDTA) tube is standard for type and screen/crossmatch at most facilities, but confirm against your own transfusion service’s specimen requirements rather than assuming; tube type mismatches are a common source of specimen rejection.
- Positive patient/specimen identification (wristband and label) supplies specific to transfusion — many facilities use a dedicated blood-bank ID band or barcode system distinct from the general hospital wristband, precisely because mislabeled type-and-crossmatch specimens are one of the most consequential identification errors in a hospital; stock the dedicated bands and labels, not a substitute.
- Tourniquets, gauze, adhesive bandages, and sharps containers at the draw station, sized for the higher per-shift draw volume a transfusion service collection area typically sees.
Leukoreduction-filter-adjacent consumables and component-handling supplies
Most red cell and platelet units a transfusion service receives today are already pre-storage leukoreduced by the collecting blood center before they arrive — the filtration itself is a blood-center manufacturing step, not something performed at the hospital bench, and this checklist stays at a general-supply level rather than specifying filter-media procurement, which is a supplier-qualification decision for the collecting facility, not a hospital transfusion-service stocking line. - Bedside or in-line leukoreduction filter sets — held in stock for the minority of cases where filtration happens at the point of administration rather than pre-storage (some platelet products, some facility-specific protocols), plus the spike/administration sets these filters attach to.
- Blood administration sets with in-line filters (the standard 170-260 micron filter built into a transfusion set) — distinct from a dedicated leukoreduction filter and required for every unit transfused regardless of leukoreduction status; stock enough that a set change between units, or per facility policy, doesn’t stall a transfusion.
- Component segments and tube-sealing/tie supplies for the sample segments retained on every issued unit — these segments are what crossmatch confirmation and post-transfusion lookback testing rely on, and running short of sealing supplies at the wrong moment turns into a documentation gap, not just an inconvenience.

Transfusion-reaction basic-response readiness
A suspected transfusion reaction is a stop-the-line event: the transfusion is halted immediately, the IV line is kept open with normal saline (not the blood product line), and a defined workup begins. What belongs on a supply checklist is making sure the physical kit a bedside nurse reaches for during those first few minutes is actually stocked and located where the reaction happens, not back at the blood bank:
- Normal saline for line-keep-open staged at or near transfusion-capable units, separate from the blood product itself — the reaction-response step of switching the line to saline only works if saline is already at hand, not a call away.
- Transfusion-reaction workup specimen tubes and post-reaction paperwork/labels — the post-reaction blood and urine specimens (and the returned unit with its administration set) that a reaction workup requires need to be collected correctly the first time; stock the specific tube types and reaction-report forms/labels your transfusion service’s workup protocol specifies, at the units level, not just centrally in the blood bank.
- Return-to-blood-bank supply kit — a biohazard bag and labeling sufficient to return the implicated unit, tubing, and specimens to the transfusion service intact, since the unit itself is part of the investigation.
- Vital-signs and basic emergency-response supplies at the point of transfusion — a working thermometer, blood pressure cuff, and pulse oximetry at minimum, since fever, hypotension, and tachycardia are the reaction signs nursing staff are screening for at first recognition; this checklist covers only that basic vitals-and-supply layer, not full anaphylaxis/code-cart contents, which belong to the unit’s general emergency-response stocking, not a transfusion-specific list.
- Patient and component identification cross-check materials at the bedside (the second-person verification/scanning step immediately before hang) — this is the single highest-value error-prevention point in the entire transfusion process, and it depends on having a working barcode scanner or verification form actually present at the bedside, not assumed to be there.

Frequently asked questions
Does a hospital transfusion service need to stock its own leukoreduction filters?
Usually no. Most red cell and platelet units arrive pre-storage leukoreduced from the collecting blood center, so the filtration step and its filter-media procurement happen upstream, at the blood center, not at the hospital bench. What’s the difference between the standard blood administration set filter and a leukoreduction filter?
The standard in-line filter on every blood administration set (typically 170-260 micron) removes clots and larger debris from any unit being transfused — it’s required regardless of leukoreduction status. A leukoreduction filter is a separate, finer depth filter specifically designed to trap white blood cells, used either pre-storage at the blood center (the common case today) or, less often, at the bedside. They’re not interchangeable and stocking one doesn’t substitute for the other.
Should butterfly needles or straight needles be the default for transfusion-service draws?
Neither is a universal default — the right device depends on vein access, draw volume, and patient population, which is why this checklist stocks a range of both rather than standardizing on one.
