Doctor in Mayagüez by Video: ERs, Urgent Care and Pharmacies
If you need a doctor in Mayagüez at night, Mayagüez Medical Center and Hospital Perea both run 24-hour ERs, and Perea sees adults and children. The Walgreens pharmacy counters on Ave Hostos close at 10 p.m. For a cold, fever, UTI or refill, Dr. Diana Nieves Castro, MD, sees patients of all ages by video while they are in Puerto Rico.

Emergency rooms in Mayagüez: Mayagüez Medical Center and Hospital Perea, 24/7
In an emergency call 911. Each ER was checked on the hospital's own site or in federal hospital data (CMS). None is affiliated with us.
Mayagüez Medical Center (Dr. Ramón Emeterio Betances), Route 2, Bo. Sábalos. Phone 787-652-9200; adult ER ext. 72809, 72813, 72334, 78423 or 72746. Open "de lunes a domingo, 24 horas" (every day, 24 hours). Its directory lists only an adult ER; we found no pediatric ER.
Hospital Perea (Metro Pavia), Calle Dr. Basora #15. Per its website, a 24/7 ER for adults and children, with 24/7 pediatric consultations (it is not on Metro Pavia's pediatric ER list). Phone unconfirmed: its website gives 787-832-0101, federal records give 787-834-0101. If one does not answer, try the other.
Hospital Bella Vista (Adventist), Route 349 Km 2.7, Cerro Las Mesas. Phone 787-652-6045, per CMS, which lists an ER; its hours are unconfirmed. We found no working official website.

What happened to Hospital San Antonio? It is now MMC San Antonio
In April 2025, Primera Hora reported that Mayagüez Medical Center would be the new operator of Hospital San Antonio on a 35-year contract, committed to keeping its neonatal and pediatric intensive care units open. It is now MMC San Antonio: 18 Calle Dr. Ramón E. Betances N, 787-834-0050. Its website advertises "Emergencias 24/7" (24/7 emergencies) with pediatric doctors.
Since June 2026 the hospital has been under a roughly 18-month renovation (patient rooms, pediatric ICU, delivery room and operating rooms), according to El Nuevo Día. The article does not say whether the ER is affected, so call before you go.
Urgent care in Mayagüez: Salus Plaza Sultana closes at 9 p.m.
Salus Urgent Care Mayagüez, at Plaza Sultana, 787-792-5389, is open daily 7 a.m.–9 p.m. It is not a hospital ER and it is not open overnight. It suits a cut that may need stitches or a sprain that needs an exam. After 9 p.m., or for anything serious, go to a hospital ER or call 911. For anything that is not an emergency, the doctor can see you by video. If unsure, read where to go when you are sick.
Is there a 24-hour pharmacy in Mayagüez? The store yes, the counter no
Two Walgreens stores on Ave Hostos stay open all night, but their pharmacy counters close at 10 p.m., the latest on the west coast. From the Walgreens locator and the Farmacias Caridad directory:

Nearest 24-hour pharmacy counter: Walgreens, 2979 Ave Emilio Fagot, Ponce, 787-841-7400, 36.3 miles per the locator (not a drive time). Closed 1:30–2 a.m. for a meal break.
On a Sunday, Caridad Medical Emporium's counter is open until 7 p.m. Call first; hours change.
Sick at night in Mayagüez: after 10 p.m.
Mayagüez nights have two cut-offs: Salus closes at 9 p.m. and the Ave Hostos pharmacy counters at 10 p.m. After that, no counter is open anywhere on the west coast. Work through it in this order:
- Is it an emergency? Chest pain, shortness of breath, stroke signs or a serious wound: call 911. Mayagüez Medical Center and Perea are open 24 hours.
- A very sick child? Perea sees adults and children around the clock. MMC San Antonio advertises 24/7 emergencies with pediatric doctors but is under renovation, so call first. For anything that is not an emergency, the doctor sees children of all ages by video: see video visits for children.
- Prescribed something in the evening? Get to Ave Hostos before 10 p.m. After that, the nearest open counter is in Ponce.
- Something over the counter? The Walgreens stores at 2097 and 992 Ave Hostos are open all night.

Doctor in Mayagüez by video: how we care for you
Dr. Diana Nieves Castro, MD, is a primary care physician licensed in Puerto Rico, with several years of emergency-department experience. She sees adults and children of all ages by video, in English or Spanish. She evaluates, diagnoses and treats; if needed, she gives you a lab order for a lab near you, such as a strep, urine or blood test. If appropriate, the doctor prescribes and we send your prescription to your pharmacy. We do not prescribe controlled substances.
We send you a secure video link; it works on your phone, tablet or computer. You just need to be in Puerto Rico during the visit. Your card is charged when you request the visit, with a full refund if it can't happen, and a doctor's note after the visit is included.

UPRM students: campus health services and the under-21 rule
The University of Puerto Rico at Mayagüez (UPRM) Department of Medical Services is at Boulevard Alfonso Valdés 259, 787-832-4040 ext. 3408 or 2333. Per its page, it offers consultations Mon–Fri 8 a.m.–3:30 p.m. and emergencies Mon–Fri 8 a.m.–4:30 p.m., mainly for enrolled students. It is closed at night and on weekends.
If you are in Puerto Rico, the doctor can see you by video for a sore throat, UTI symptoms, allergies, or a non-controlled prescription that ran out away from home. The age of majority in Puerto Rico is 21: if you are younger, a parent with patria potestad (parental authority) or a legal guardian signs the consent, stays on camera for the whole visit and shows photo ID. An older city, and dengue in the west
Mayagüez lost 5.5% of its population between 2020 and 2024 (73,077 to 69,044), and 27.5% of residents are 65 or over, per the Census Bureau.
