Online Doctor in West Puerto Rico: From Añasco to La Parguera
If you need an online doctor in west Puerto Rico, Dr. Diana Nieves Castro, MD, sees patients of all ages by video in Añasco, Hormigueros, San Sebastián, Las Marías, Maricao, Sabana Grande and Lajas. None of these seven towns has a hospital. For an emergency, the ERs are in Mayagüez, San Germán, Lares (Castañer) and Moca, and there is no 24-hour pharmacy counter anywhere in the west.
Is it an emergency? Call 911.
Video only
We are a telehealth-only service: we have no office or clinic in this area or anywhere else in Puerto Rico. If you need to be seen in person, this guide shows where to go.
Is this an emergency?
This is not an emergency service. Call 911 or go to the nearest emergency room if you have:
- Chest pain, shortness of breath or stroke signs
- Someone pulled from the sea who coughs a lot or is short of breath
- A sea urchin spine buried in a joint, or a sea wound that turns red and hot with fever
- Confusion, fainting or hot dry skin after time in the sun
Help lines
Seven towns, four nearby hospitals and no overnight pharmacy counter
The west splits two ways. The coastal and valley towns (Añasco, Hormigueros, Sabana Grande and Lajas) use the hospitals in Mayagüez and San Germán. San Sebastián is closest to Hospital San Carlos Borromeo in Moca. Las Marías and Maricao, in the mountains, are 8 to 11 miles in a straight line from the Mayagüez and San Germán hospitals and from Hospital General Castañer in Lares, whose site says its ER is open 24 hours, 365 days a year.
Pharmacies are another story: the nearest 24-hour counter to any of these seven towns is the Walgreens on Ave Emilio Fagot in Ponce, 24 to 40 miles away in a straight line. Five of the seven towns have no Walgreens. So for a problem that is not an emergency, a video visit with the prescription sent to your local pharmacy is often the practical choice.

How the doctor sees you in the west
Dr. Diana Nieves Castro, MD, is a general practice physician licensed in Puerto Rico, with several years of emergency-department experience. She sees adults and children of all ages, in English or Spanish. She evaluates, diagnoses and treats; if needed, she gives you a lab order for a lab near you. If appropriate, the doctor prescribes and we send your prescription to your pharmacy, whether that is the Walgreens in San Sebastián, a Caridad in Lajas or your local drugstore. We do not prescribe controlled substances.
We send you a secure video link; it works on your phone, tablet or computer. Anyone under 21 needs a parent or legal guardian on the video, with photo ID. Request a visit online 24/7 — we confirm your time by email or text. Your card is charged when you request the visit (Apple Pay or Google Pay if they appear on the payment page), with a full refund if it can't happen.
- Doctor visit, US$79: new problems and non-emergency urgent care.
- Follow-up, US$49: a refill of a medicine you already take.
- Forms, US$35: school, university or job forms.
Dengue, and how this page was checked

Verified on October 2, 2026: hospitals in CMS data, on Castañer's site and on Metro Pavia's list; pharmacy counters on each Walgreens page and the Farmacias Caridad locator; health centers in HRSA data; population from the Census Bureau. Distances are straight lines. We re-check these every quarter. See also Cabo Rojo, Mayagüez, San Germán and all areas.
