See a Doctor in Caguas by Video: Dengue, ERs and the 24-Hour Pharmacy

Salus Urgent Care Caguas, Plaza del Carmen Mall, Suite 24, 787-286-6060. Open Monday to Sunday, 7 a.m.–9 p.m. It is an urgent care center, not a hospital ER, and it is closed overnight. Salus has another center at Cayey Shopping Center with the same phone and hours.

We could not verify any other urgent care center or CDT in Caguas with late hours, so we list none. If you are unsure whether you need urgent care, an ER or a video visit, read where to go when you are sick. For anything that is not an emergency, the doctor can see you by video.

Once Salus closes at 9 p.m., Caguas is down to two hospital ERs and one pharmacy counter open all night. Work through it in this order:

Dr. Diana Nieves Castro, MD, is a primary care physician licensed in Puerto Rico, with several years of emergency-department experience. She sees adults and children of all ages, in English or Spanish. She evaluates, diagnoses and treats; if needed, she gives you a lab order for a lab near you, such as a strep, urine or blood test. If appropriate, the doctor prescribes and we send your prescription to your pharmacy, including the 24-hour counter on PR-1. We do not prescribe controlled substances.

We send you a secure video link; it works on your phone, tablet or computer. You just need to be in Puerto Rico during the visit. Anyone under 21 needs a parent or legal guardian on the video, with ID. Your card is charged when you request the visit, with a full refund if it can't happen, and a doctor's note after the visit is included.

The Puerto Rico Department of Health's weekly report for week 37 (published September 23, 2026, the latest as of September 30) counts 118 dengue cases in Caguas from January 1 to September 16. That is 12.8% of the island's 922 cases and more than any other municipio. By rate, 94.68 per 100,000 residents, it ranks fifth, behind Arroyo, Guayama, Adjuntas and Salinas. The Caguas health region totals 313 cases, the most of any region, and one of 2026's four dengue deaths was there: a 62-year-old man, in May, with serotype DENV-3. The report does not say why Caguas is so high, and we won't guess.

The CDC says warning signs usually start 24 to 48 hours after the fever breaks: belly pain or tenderness, vomiting at least 3 times in 24 hours, bleeding from the nose or gums, blood in vomit or stool, or extreme tiredness or irritability. Caguas is the island's fifth most populous municipio (127,244 residents in the 2020 Census). PR-52 links it to San Juan to the north and to Cayey and Ponce to the south, and PR-1, PR-30 and PR-172 cross the valley. Its malls are Plaza del Carmen, Plaza Centro and Las Catalinas; downtown is the Plaza de Recreo Santiago R. Palmer, and on the edge of town the Jardín Botánico y Cultural William Miranda Marín (per Wikipedia, a secondary source).

During Hurricane María in 2017, a Caguas rain gauge recorded 37.90 inches, the highest total on the island, according to the National Hurricane Center's report.
