See a Doctor in Toa Baja, Levittown and Toa Alta by Video
If you need a doctor in Toa Baja, Levittown or Toa Alta, know that neither municipio has a hospital: the nearest verified round-the-clock ER is SIM in Toa Alta town, and the hospitals are in Bayamón. If it is not an emergency, skip the drive: Dr. Diana Nieves Castro, MD, sees patients of all ages by video while they are in Puerto Rico. In an emergency, call 911.

No hospital and no CDT in Toa Baja: where do you go?
Federal hospital data (CMS) lists no hospital in Toa Baja, Levittown or Toa Alta. And in November 2016 Toa Baja sold its CDT (the municipal diagnostic and treatment center) to Metro Toa Baja, Inc. for $4,251,000 to pay down municipal debt, according to Primera Hora. We could not verify any ER or urgent care center in Levittown.
What Toa Baja does have: Morovis Community Health Center runs a clinic at Carr. 863 Km 1.0, Bo. Pájaros, 787-862-3000, open 56 hours a week per the federal health-center directory (HRSA). It offers primary care and does not advertise an ER.
That leaves three options: SIM's ER in Toa Alta, the hospitals in Bayamón, or a video visit when it is not an emergency. Not sure which? Read where to go when you are sick.

Toa Alta's 24/7 ER (SIM) and urgent care at CSI Toa Alta II
Salud Integral en la Montaña (SIM) runs the only round-the-clock ER we could verify in Toa Baja or Toa Alta. It is not affiliated with us.
SIM, CSI Toa Alta (emergency room), 16 Calle Antonio R. Barceló, Toa Alta. Phone 787-870-3322. Open 24 hours, 7 days, for adults and children, per SIM; the HRSA directory lists it at 168 hours a week.
SIM CSI Toa Alta II, Carr. 165 Km 4.5, Bo. Quebrada Cruz. Phone 787-545-8808. Regular hours Mon–Fri 8 a.m.–4:30 p.m., extended Mon–Fri until 9 p.m. and Sat 8 a.m.–6 p.m. It is not a 24-hour ER: at night and on Sundays, go to the ER in town or call 911.
SIM CSI Toa Alta, Calle José de Diego at Barceló 23, 787-870-0008: primary care Mon–Fri 8 a.m.–4:30 p.m.
Nearest hospitals: Bayamón

Bayamón Medical Center (adults), Carr. 2 Km 11.7, Bayamón. Phone 787-620-8181. Advertises an adult ER open "24/7, 365 days".
Puerto Rico Children's Hospital (children), on the same stretch of PR-2, Bayamón. Phone 787-474-8282. Its site says it provides services 24 hours a day, 7 days a week, and that a child must be accompanied at all times by a parent or authorized representative.
Levittown and Toa Alta pharmacies: a 24-hour store is not a 24-hour counter
The Walgreens on Ave Los Dominicos keeps its store open all night, but its pharmacy counter closes at 10 p.m. From each store's page on the Walgreens locator and the Farmacias Caridad directory:

None of the island's five 24-hour pharmacy counters (all Walgreens) is in Toa Baja or Toa Alta. The nearest is Walgreens, 4699 Route 5, Bayamón, 787-799-4699, 4.3 miles from Toa Alta per the locator (not a drive time), closed 1:30–2 a.m. for a meal break. Another is on PR-841 in Guaynabo. Call before you go; hours change.
Sick at night in Levittown, Toa Baja or Toa Alta
At night, what matters here is the clock: CSI Toa Alta II closes at 9 p.m. and the Los Dominicos pharmacy counter at 10 p.m. Work through it in this order:
- Is it an emergency? Chest pain, shortness of breath, stroke signs or a serious wound: call 911. SIM's ER in Toa Alta town and Bayamón Medical Center are open 24 hours.
- A very sick child? SIM's ER sees children around the clock, and Puerto Rico Children's Hospital is on PR-2 in Bayamón. For anything that is not an emergency, the doctor sees children of all ages by video: see video visits for children.
- Need a prescription filled? Before 10 p.m., the Walgreens counter on Ave Los Dominicos. After that, Walgreens, 4699 Route 5 in Bayamón, open all night except 1:30–2 a.m.
- Something over the counter? The Walgreens store on Ave Los Dominicos is open 24 hours.

Doctor in Toa Baja by video: how we care for you
Dr. Diana Nieves Castro, MD, is a primary care physician licensed in Puerto Rico, with several years of emergency-department experience. She sees adults and children of all ages by video, in English or Spanish. She evaluates, diagnoses and treats; if needed, she gives you a lab order for a lab near you, such as a strep, urine or blood test. If appropriate, the doctor prescribes and we send your prescription to your pharmacy. We do not prescribe controlled substances.
We send you a secure video link; it works on your phone, tablet or computer. You just need to be in Puerto Rico during the visit. Anyone under 21 needs a parent or legal guardian on the video, with ID. Your card is charged when you request the visit, with a full refund if it can't happen, and a doctor's note after the visit is included.

Floods and the Río La Plata: prepare your medications
During Hurricane María the Río La Plata rose more than 11 feet on September 20, 2017, and 2019 flood maps describe Toa Baja as "extremely vulnerable" to flooding. The area around Lago La Plata in Levittown flooded too, and in Toa Alta María caused many landslides (per Wikipedia, a secondary source).
If you live in a flood zone, keep a list of your medications and doses, photos of the labels and a spare supply in a waterproof bag you can grab. If floodwater takes your non-controlled medications, the doctor evaluates you by video and, if appropriate, prescribes replacements and we send your prescription to your pharmacy. Levittown, Toa Alta and dengue: who lives here
Levittown, which Levitt & Sons began building in 1963 around the Bulevar de Levittown and Ave Los Dominicos, had 25,591 residents in the 2020 Census; PR-165 links it to Toa Alta, Cataño and Dorado (per Wikipedia). Toa Alta has the youngest population of the metro areas we cover: only 18.5% are 65 or older (Toa Baja: 23.3%), per the Census Bureau. That means many families with kids, and with no hospital in town, a child's fever at night often means a drive to Bayamón or to SIM's ER.
According to the Puerto Rico Department of Health's weekly report for week 37 (published September 23, 2026, the latest as of September 30), Toa Baja has 27 dengue cases so far in 2026 and Toa Alta 17. Standing water after rain breeds mosquitoes, so empty containers and use repellent. Dengue warning signs mean the ER; see our dengue guide.
